Common mistakes to avoid

Enter values for both eyes (OD & OS) if provided
CYL and AXIS must be entered together
If you don’t have CYL, leave AXIS blank
AXIS must be between 0 and 180
SPH and CYL can be 0.00
Only fill ADD if it appears on your prescription

Quick tips

“–” means nearsighted, “+” means farsighted
A mix of “+” and “–” SPH values is uncommon — please double-check
CYL is usually negative — confirm if yours is positive
ADD is usually the same for both eyes

OD (Right eye) | OS (Left eye)

OD stands for oculus dexter (right eye), and OS stands for oculus sinister (left eye).

Enter the values for each eye exactly as shown on your prescription.

Sphere (SPH)

Also known as Power (PWR), this indicates the strength of your prescription.

A minus (–) value means nearsighted, and a plus (+) value means farsighted.

Enter the value exactly as shown, including the + or – sign. SPH can be 0.00.

Cylinder (CYL)

Used to correct astigmatism, where different parts of the eye require different levels of correction.

Only fill this if it appears on your prescription. If you don’t have CYL, leave it blank or select 0.00. If you enter CYL, you must also enter AXIS.

Axis

A number between 0 and 180 that determines the orientation of the Cylinder (CYL).

Only fill this if you have a CYL value. If CYL is blank or 0.00, leave AXIS blank.

ADD

Additional magnifying power for near vision, typically used for reading or multifocal lenses.

Only fill this if it appears on your prescription. ADD values are usually the same for both eyes.

Make sure all values match your prescription exactly, including + and – signs.
